Travel to Jordan Declaration Form - Arriving through land borders

Traveling instructions:

- For Jordanians: Please upload a picture of your passport only.
- For non-Jordanians: Please upload a copy of your passport, and your residency in the GCC, or the United States or Europe.
- For nationalities that require prior approval before arrival please upload your visa to Jordan.
- The PCR test should be done within maximum 72 hours prior to departure.
- All travelers must complete a health declaration and locator form, obtain health/travel insurance (For Non Jordanians), and visit to complete the required travel declaration form and obtain their personal QR code which is mandatory for entering Jordan.
- Passengers are required to present the applicable documents upon arrival. This includes the QR code, a negative PCR test result from the country of departure, and the vaccination certificate (for vaccinated passengers).
- Children under 5 years old are not required to apply on the platform.
- In case of a positive result, travellers must commit to adhere to home quarantine for a period of 14 days and do the (PCR)(COVID19) test on the fourteenth day of home quarantine at their own expense.
- Travelers with a Jordanian vaccination certificate whose second dose of vaccination has been taken before at least 14 days, and who return to Jordan within 10 days from the date of their departure are exempted from bringing a negative PCR test result from the country of departure.

Note 1: You will be required to undergo a COVID-19 test upon arrival in Jordan at your own expense of 28 JD (approximately 39), children below 5 years of age are exempted – Your application will not be processed unless payment is made., please make payment now.

Note 2: Upon successful completion and submission of this form, you will receive a confirmation code via email, which must be shown at the border crossing.

Note 3: Evidence of negative PCR tests conducted from the country/region of departure and confirmation code (see Note 1) must be presented at the border crossing for each traveller.
